PCHI exists to assist communities in creating a sustainable community-based care coordination network that supports their under-resourced residents and connect them to services, track outcomes, and contract with payers that directly tie payment to those outcomes. Through the Pathways Community HUB Institute℠ Model (PCHI℠ Model), communities are provided with the framework and support to build a robust, complete network of care. The PCHISM Model is focused on the whole person and working in in partnership across multiple community stakeholders to align efforts and resources to reach the common goal of good health for all. PCHI knows first-hand the impact that a community-designed care coordination network can have on individuals and their families. Today, the PCHI℠ Model is being implemented in 40 communities in 17 states and counting.
