Case Manager II - Casa De La Mision

PAY: $28.00-$32.00/hour DOE Works with the resident to develop appropriate case plans and goals and provide on-going assessment and support. Assists the resident to utilize community resources to meet individual needs. Works with residents to identify their skills and abilities to promote personal growth. Caseload is larger than that of a Case Manager I; serves as mentor to Case Manager I.
