Overview

The SOC (Start of Care) Nurse is responsible for providing comprehensive nursing assessments and initiating home care services for patients transitioning into care. The SOC Nurse conducts Start of Care visits, develops and implements individualized plans of care, coordinates multidisciplinary services, and ensures compliance with regulatory, agency, and payer requirements.

The role requires strong clinical judgment, care coordination, documentation, and communication skills to support safe and effective patient outcomes. Works under moderate supervision. Qualifications

BLS certification preferred. Current New York State Registered Nurse license and registration required Education: Associate's Degree in Nursing required. Bachelor's Degree in Nursing from an accredited nursing program preferred Work Experience: Minimum one year of recent clinical nursing experience in home care, medical-surgical, acute care, or related setting.
