Licensed Mental Health-*Therapist-Crisis Navigator*-(Cuyahoga County)

Crisis Navigator will be doing important and meaningful work that will bridge the gaps in SUD, Mental health, and physical health care to improve and save lives. Based on a person and family-centered plan of care, the Crisis Navigator will be the pillar of our crisis management services, stabilizing adults and children in their time of need until their needs can be met by our long-term care clinicians.

The Crisis Navigator position will be providing social services, including behavioral and mental health assessments, social determinants to health and wellness, and educational systems advocacy, as necessary to facilitate the wellness and recovery of the whole person.
