Become a part of our caring community The Field Care Manager, Behavioral Health 2 assesses and evaluates members’ behavioral health and psychosocial needs to achieve and/or maintain optimal wellness. This role guides members and their support systems in navigating behavioral health, medical, and community resources to support improved health outcomes.
The Field Care Manager, Behavioral Health 2 employs evidence-based strategies to manage members’ behavioral, physical, environmental, and psychosocial needs.
Responsibilities include
identifying barriers, coordinating care, monitoring progress, and adjusting interventions to support outcomes. Core Responsibilities Conduct comprehensive behavioral health assessments Develop member-centered care plans with measurable outcomes Coordinate services across providers and community resources Support reentry and care transitions, including incarceration discharge Conduct field visits and telephonic outreach Field-Based Responsibilities Visits occur in homes, community settings, hospitals, shelters, and correctional facilities .
These visits are essential to the role. Work Environment Field-based and telephonic Must reside in Eastern Kentucky Travel required: Up to 75%. (30–40 miles) Quarterly meetings in Louisville, KY Use your skills to make an impact Required Qualifications Must reside in the Eastern Kentucky area Master's degree in a Behavioral Health or related field.
Active, unrestricted Kentucky licensure in one of the following: Licensed Clinical Social Worker (LCSW) Licensed Professional Clinical Counselor (LPCC) Licensed Marriage and Family Therapist (LMFT) Other independent Kentucky behavioral health license with at least 1 year of post-degree clinical experience OR Active, unrestricted Kentucky Registered Nurse (RN) license, with a bachelor's degree in nursing, and a minimum of 3 years of behavioral health experience.
2+ years of case management or care management experience. Experience working with adult populations.
Preferred Qualifications
Experience working with the Kentucky Medicaid population. Managed care experience. Certified Case Manager (CCM) certification. Experience supporting individuals with Severe and Persistent Mental Illness (SPMI) and/or Substance Use Disorder (SUD). Experience working with high-risk populations. Knowledge and application of motivational interviewing and trauma-informed care practices.
Experience in behavior change, health promotion, wellness coaching, and member engagement. Bilingual in English and Spanish, with the ability to speak, read, write, interpret, and explain documents in Spanish. Additional Information Schedule/Time Zone: Monday through Friday, 8:00 AM – 5:00 PM with flexibility to work overtime as needed.
Work Location: Must reside in the Eastern Kentucky area. Work Style: Field Travel Requirements: Up to 75% travel, approximately 40 miles per day, with quarterly meetings in Louisville, KY. TB Statement: This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driving Statement : This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$65,000 - $88,600 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
