  • No NV state income tax What you'll do Deliver comprehensive primary care to Care. More ISNP members with complex chronic conditions in SNF, LTC, and institutional settings. Conduct admission evaluations, routine follow-ups, and acute visits. Develop individualized care plans including chronic disease management and advance care planning. Coordinate transitions of care: hospital discharges, readmission prevention, medication reconciliation. Collaborate with nurses, care managers, social workers, specialists, and facility staff. Support STARs/HEDIS quality outcomes and reduce avoidable ER visits and readmissions.
