Registered Nurse, Care Manager-PACE (Temporary)

Neighborhood Healthcare is seeking a compassionate and skilled Registered Nurse, Care Manager to join our PACE Program, a Program of All-Inclusive Care for the Elderly. In this vital role, you will conduct patient assessments, develop & coordinate individualized care plans, and monitor ongoing care to ensure it remains medically appropriate.

You’ll work closely with an Interdisciplinary Team (IDT) to manage transitions from hospitals or skilled nursing facilities and implement skilled nursing services. This is a full-time temporary position, expected to last about 3 months. Schedule: Monday-Friday, 8am-5pm. This position is onsite.
