As our Transitional Care Coordinator , you’ll serve as a trusted resource for hospitals, physicians, assisted living communities, independent living communities, memory care communities, adult foster homes, skilled nursing facilities, and other healthcare partners throughout the Rogue Valley.
You’ll help patients navigate the transition into Home Health and Hospice while building meaningful partnerships that strengthen our community and expand access to compassionate, life-changing care.
