Job description
The Chronic Care Manager (CCM) provides clinically driven, telephonic care management services to a panel of complex patients within the ambulatory setting. This role is responsible for implementing the care management process, developing and maintaining individualized care plans, and supporting patient-centered care in alignment with regulatory guidelines.
The CCM works with patients, families, providers, and interdisciplinary teams to monitor chronic conditions, address both medical and psychosocial needs, and support improved health outcomes.
