Job description
Manage and supervise all aspects of service for individuals with intellectual disabilities and/or autism spectrum disorder. Work within a clinical team to develop, plan, and implement case management, therapy, and community/residential assistance. The Service Coordinator will specialize in supporting adults with developmental disabilities and significant medical needs.
This would support an aging population of clients including end of life care. These clients require many specialized care procedures and medication administration. This Service Coordinator would be in contact with external partners like the hospital, medical teams, families, guardians, shared living providers, internal nurses, etc.
