Job description

Healthcare United is seeking a full-time Lead Primary Care Physician to join an established value-based healthcare organization in Cincinnati, Ohio. This position combines direct patient care with clinical leadership in a multidisciplinary outpatient setting. The Lead Physician will manage a panel of medically complex and socially vulnerable patients while mentoring physicians, advanced practice providers, and clinical team members.

The organization’s care model focuses on improving outcomes, addressing barriers to care, and reducing avoidable emergency room visits, hospitalizations, and readmissions. Physicians are supported by integrated behavioral health, pharmacy, care management, social services, and clinical support teams.

Responsibilities

  • Provide comprehensive primary care to medically complex patients ages 16 and older
  • Diagnose and manage acute and chronic medical conditions
  • Develop individualized, evidence-based treatment plans
  • Provide preventive care, medication management, and same-day urgent visits
  • Lead, mentor, and support physicians and advanced practice providers
  • Collaborate with behavioral health, pharmacy, care management, and social services
  • Support the management of substance use disorders, behavioral health conditions, and infectious diseases
  • Monitor quality, utilization, and patient outcome measures
  • Help reduce avoidable emergency room visits, hospitalizations, and readmissions
  • Participate in clinical quality improvement and population health initiatives
  • Maintain accurate and timely clinical documentation
  • Participate in a rotating call schedule Requirements
  • MD or DO degree
  • Board certification in Family Medicine or Internal Medicine required
  • Infectious Disease or Addiction Medicine physicians may be considered with recent primary care experience
  • Active Ohio medical license or eligibility to obtain
  • Active DEA registration or eligibility to obtain
  • Several years of primary care experience required
  • Clinical leadership, mentorship, or supervisory experience preferred
  • Experience with value-based care, community health, FQHC, or underserved populations strongly preferred
  • Comfortable caring for medically and socially complex patients
  • No new graduates
