Physical Therapist (PT) / Occupational Therapist (OT) – Home Health Visits | Pasco and Pinellas Counties, FL

Matrix Home Care is seeking experienced Physical Therapists (PTs) and Occupational Therapists (OTs) to provide home health visits throughout Pasco and Pinellas Counties. We are looking for compassionate, reliable, and professional therapists who are comfortable working independently in the field and providing quality care to patients in their homes.

The PT/OT will provide therapy services to patients in the home setting, complete evaluations and visits as assigned, develop and update treatment plans, educate patients and caregivers, and ensure accurate and timely documentation.

Responsibilities

Complete therapy evaluations and follow-up visits in the home setting Develop individualized treatment plans based on patient needs Provide skilled therapy services in accordance with the plan of care Educate patients and caregivers on safety, mobility, exercises, activities of daily living, and home programs Communicate with the care team regarding patient progress and changes in condition Complete timely and accurate documentation Maintain compliance with home health standards and agency requirements Schedule: Full time/ Per diem / visit-based Flexible scheduling based on patient needs and therapist availability Coverage needed throughout Pasco and Pinellas Counties Requirements Active Florida Physical Therapist or Occupational Therapist license Home health experience preferred Strong assessment, documentation, and communication skills Ability to work independently in the field Reliable transportation, valid driver’s license, and auto insurance required Must be professional, dependable, and patient-focused Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
