Hospice: RN Case Manager (South Central) - $3000 Sign on Bonus

The Hospice RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The Hospice RN Case Manager builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Salary Range:  $88,000-$94,600 annually depending on experience Service Area: South Central (coverage area: St. Louis Park, Richfield, Bloomington, Burnsville, Lakeville & surrounding). Plus, mileage reimbursement of 72.5 cents per mile. Schedule: 1.0 FTE, Monday – Friday 8:00am-5:00pm(with every 5th weekend rotation or 5 weekend shifts/quarter, 2 holidays/year and no on-call)
