Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Horizon House manage and supervise the operations and staff, at multiple assigned service sites, where Home and Community-Based Services (HCBS) (for adults with cognitive impairment, mental health diagnoses and/or behavior issues) are delivered
