Job description
As a Transitions Caseworker, you will provide clients the support they need in developing the skills to move from homelessness to housing by assessing their strengths and barriers to housing stability and recovery. Working with a housing first, strength-based approach and recovery-oriented lens to proactively engage with unhoused individuals.
This work includes supporting clients in overcoming the barriers they may have for housing and providing the support they need to move towards independent living which could include distress and crisis intervention.
