The Comprehensive Class Member Transition Program is an Illinois State grant funded program designed to support Williams and Colbert Class members residing in Long Term Care Facilities into independent housing with wraparound community supports. The program offers an array of services with the diverting agency such as community mental health supports through an integrated healthcare model and long-term housing supports through transitional programs or subsidy services. The Housing Coordinator will coordinate housing efforts for the CCMTP team by developing and maintaining relationships with landlords and private management companies who will work with our clients.
