Case Manager I - BHS - Healing & Recovery Center - Muskogee

The purpose of the position is to ensure a holistic, whole-person approach to providing patient support, referrals and resource links. The Case Managers/Coordinator will be responsible for maintaining contact and follow up care to patients identified. Case Managers/Coordinator will be assigned to a BH location, but also be asked to float to other clinics at times.
