CCBHC Targeted Case Management Specialist

The CCBHC Targeted Case Management Specialist provides case management to specific CCBHC clients to enhance access to critical behavioral, medical and social services for individuals with complex behavioral health challenges. The Targeted Case Management Specialist is responsible for identifying, linking, and monitoring services for individuals with SMI (Serious Mental Illness) or SED (Serious Emotional Disturbance) and Substance Use Disorders for time-limited episodes of care.
