Case Manager - Jail Reentry

$55K annual salary, full benefit package and up to 34 paid days off 1st year! Case Manager will provide risk, mental health and substance abuse screening services to criminal justice defendants with the goal of engaging into treatment prior to release from incarceration. Services provided primarily at the David L. Moss Criminal Justice Center, however; some services may take place at other agency locations.

Services predominantly delivered to adult population with serious mental illness, substance abuse, and/or co-occurring diagnosis. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
