What You’ll Do Deliver high-quality physical therapy care in patients’ homes Develop, implement, and manage individualized care plans Collaborate with a multidisciplinary care team Maintain timely and accurate documentation Travel locally to patient residences (~20% of your day) What We’re Looking For Bachelor’s or Master’s degree in Physical Therapy (required) Active Physical Therapist license in the state of practice Minimum of 1 year of healthcare experience Valid driver’s license and reliable transportation Strong time management and self-scheduling skills Comfortable using tablets, smartphones, and EMR systems Commitment to meeting documentation deadlines A compassionate, patient-centered approach to care
