Job description
Play a lead role in educating community members on health promotion and disease prevention activities. Develop and coordinate projects that directly respond to community needs and priorities. Conduct community outreach and work closely with residents, community shareholders and Community Health Promoter Lead (CHP) to identify and address health barriers and disparities.
This position is not eligible for a hybrid work schedule.
