If you are passionate about working with teens and young adults, this position could be right for you. As a Transition Facilitator, you will coach, support and help young adults (ages 16.5 to 22) with emotional, behavioral or mental health issues as they transition from the children’s behavioral health system of care into adulthood or to the adult system of care.
You will provide guidance and support in the areas of employment, education, housing, social and family relationships, independent and daily living, finances and budgeting, decision making, emotional and physical health and wellbeing, community life, and more using principles of the nationally recognized Transition to Independence Process program.
