Job description
We are seeking a dedicated Transitional Care Coordinator (TCC) to join our fast-paced Emergency Department team. This role focuses on supporting patients during critical transitions of care to ensure continuity, safety, and improved outcomes. The ED TCC will help with assessing social needs, facilitate referrals, and coordinate transportation and follow-up care.
This role works closely with social work and population health teams to address barriers to care, promote health equity, and connect patients to essential resources.
