Registered Nurse (RN) Case Manager – Hospice Canonsburg, PA & Surrounding Area We are seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to join our hospice care team. In this role, you will play a vital part in coordinating care and supporting patients and their families through every stage of the hospice journey.
You’ll work alongside a collaborative and supportive team, delivering high-quality, patient-centered care while helping ensure comfort, dignity, and continuity of care.
Key Responsibilities
Manage and coordinate comprehensive hospice care for assigned patients Collaborate with interdisciplinary healthcare teams to develop and implement care plans Provide clinical leadership, guidance, and support to nursing staff Monitor patient progress and adjust care plans as needed Ensure accurate and timely documentation in compliance with care standards Schedule & Work Expectations Monday – Friday, 8:00 AM – 5:00 PM Productivity: 6 units per day / 30 units per week On-call rotation: 1 weekend per month 1 evening per week Territory Primary coverage in Canonsburg, PA Travel within approximately a 15–25 mile radius of the community Local travel required across multiple counties What We Offer Supportive and collaborative team environment Opportunity to make a meaningful impact in hospice care Stable, full-time schedule with manageable on-call rotation
