Supervisor of Chronic Care Management

The Chronic Care Management (CCM) Supervisor provides both direct care management services and operational oversight of the Chronic Care Management program. This role is responsible for supervising CCM staff, ensuring regulatory compliance, optimizing workflows, and driving program growth and innovation. The CCM Supervisor maintains a caseload while leading a team of Chronic Care Managers.

This position requires a forward-thinking, solutions-oriented leader who can evaluate program performance, identify opportunities for improvement, and implement best practices to enhance patient outcomes and organizational success. Collaborates with providers, leadership, and interdisciplinary teams to ensure high-quality, compliant, and patient-centered care management services across all sites.
