Pace Care Coordinator Float RN (Merced, Modesto, and Stockton Pace Centers

Under the supervision of the Medical Director, Director of Center Operations and Clinical Supervisor, provides clinic nursing services including assessing participants, participates in the interdisciplinary team and care planning, implements care coordination, evaluates participant care and medical records, and provides clinical leadership to the clinical care team at the location needed.

This position will work at our PACE Centers in Merced, Modesto, and Stockton (once operational). Schedule is Monday – Friday, 8:00am – 5:00pm, with flexibility to work overtime.

Compensation

$50.16 - $58.65 an hour, plus daily $18.00 bonus for working scheduled days
