Job description
The RN acts as Case Manager for an assigned caseload of patients/families, and provides skilled nursing care in accordance with the interdisciplinary team plan of care, and under the direction of the Hospice physician or physician designee to the patient/family/caregiver. The care provided shall be in compliance with regulations as established by the State Board of Registered Nursing and the policies of Hospice of Northwest Ohio.
