Pace Adult Gerontology Nurse Practitioner (Stockton Pace Center)

The PACE Adult-Gerontology Nurse Practitioner (AGNP) provides high-quality, compassionate medical care to participants of the Golden Valley Health Centers (GVHC) PACE program. This position works under the direct supervision of the PACE Medical Director and/or designated primary care physicians and is responsible for assessments, treatment planning, coordination, and delivery of care in the clinic, day center, homes, and residential settings. 

Location and Schedule: Central Valley Stockton PACE Center, Monday - Friday, 8:00am-5:00pm. Flexibility to work scheduled evening hours, and when appropriate, act independently as a solo provider of services under a physician supervisor.

Compensation

$170,000 - $197,000 Annual Salary; Sign On Bonus and Relocation Assistance Eligible
