Forensic Care Coordinator

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a Forensic Care Coordinator to support individuals participating in OARS, Conditional Release, and Competency Restoration programs. This community-based role provides person-centered case management, service coordination, advocacy, and collaboration with treatment teams, courts, correctional systems, community providers, and natural supports.

Responsibilities include

assessments, service planning, documentation, care coordination, and court-related responsibilities as needed. Daily travel throughout Dane, Sauk and surrounding counties is required. Primary schedule is Monday–Friday with occasional evenings and weekends based on client needs.
