County Agency Social Worker

DVHHS is currently has a full-time position for a County Agency Social Worker. DVHHS serves Cottonwood and Jackson Counties in Minnesota. This position is responsible for conducting assessments and providing on going case management and crisis intervention to individuals experiencing health challenges, requiring a high level of skill and expertise.

This role involves managing judicial processes and maintaining consistent communication with individuals, community partners, and service delivery providers. The position requires the ability to meet strict deadlines, develop support plans, implement intervention strategies, and provide guidance to paraprofessional staff.

This position will be required to provide on-call services, and callback assistance on evenings, weekends, and holidays.
