Youth Care Coordinator

$55K annual salary! Full benefit package with up to 34 paid days off 1st year! Family & Children’s Services now hiring a skilled Youth Care Coordinator to provide targeted support services and Youth-centered Systems of Care (SOC). SOC coordinates mental health and community services for needs of youth/ families impacted by trauma by preventing out-of-home placements and encircling families with support and resources.

Our Care Coordinators work with children in their homes/communities while incorporating the entire family in treatment options. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
