Job description
This position involves providing intensive case management in the community to help clients maintain their transition from homelessness to long-term stability. Working from a harm reduction, trauma-informed, and client-centered approach, you will support individuals navigating mental health and addiction challenges. You will work with the clients through regular assessments and goal setting as well as collaborating with outreach services and program referrals within the community to build a system of community supports.
