Care Coordinator- Hope Team

$58,000/year Salary! Complete benefit package and generous PTO! Hope Team provides integrated medical and behavioral health care to high risk adult SMI clients in a community based setting. Position provides advocacy, linkage, referral, individual and group education, resource acquisition services, care management and coordination services to clients in Hope program in a client-centered and culturally responsive way.

Provides a point of contact and work in partnership with Hope staff and partners as part of a multidisciplinary team. Family & Children's Services utilizes the Certified Community Behavioral Health Clinic (CCBHC) model of care. As a member of a multi-disciplinary care team you will successfully support clients by providing whole person care with this transformative model.
