Medical Assistant Coordinator - Family Medicine

Provides administrative support to division and clinical support for the Maryland Primary Care Program (MDPCP) and other Value Based Performance Programs. Assists providers in the prevention, management of chronic disease, and preventing unnecessary hospital utilization by, including but not limited to, conducting SDOH patient screenings, scheduling post ED/Inpatient discharge follow-up appointments, and depression screenings.

PCMH reporting. EPIC addendums for Care. First reporting; and other duties assigned.
