Case Manager, Assertive Community Treatment (ACT)

CNS Healthcare is looking for a full-time ACT Case Manager to join our dynamic and fast-paced team. The ACT Case Manager will work with individuals and groups to promote optimum mental health. In addition, helps individuals deal with addictions and substance abuse, self-esteem, and issues associated with mental and behavioral health.
