Family Coach - Permanent Supportive Housing

Metropolitan Family Services’ permanent supportive housing program supports individual and families with disabilities to achieve housing stability. This position provides home and community based one-on-one services to clients that have recently moved into permanent housing. This position provides a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy to meet an individual’s and family’s comprehensive needs through communication and available resources.
