Stoughton Health has an excellent opportunity for an RN Case Manager to join our Medical- Surgical team on a Part-Time basis. The RN Case Manager responsible for carrying out many duties that directly affect customer service, quality care, and hospital reimbursement. This is a multi-focal role involving review of benefits for planned admissions, concurrent review of charts for medical acuity, coordination of admissions to supportive care, participation in discharge planning within the multidisciplinary care team, assistance in assignment of DRG's, and contacts with hospital departments and outside agencies.
