Job Description Swope Health is seeking a compassionate and mission-driven Community Health worker to be a part of a team of caregivers addressing social determinants of health, and focusing on reducing financial barriers that impact access to care. The Community Health Worker assists individuals and families in enrolling in public benefits such as Medicaid and the Affordable Care Act(ACA) marketplace, connecting them to critical resources, and providing financial literacy education.
