Job description
The Pharmacist supports Monogram’s medication optimization program by conducting comprehensive medication reviews, identifying drug therapy concerns, and communicating clinical recommendations to treating providers. The pharmacist conducts telephonic consultations with medical providers when medication changes appear necessary and speaks with patients about medication use concerns.
The pharmacist collaborates with interdisciplinary clinical staff to integrate medication optimization into Monogram's care model and ensures that field clinicians are executing goal-directed medical therapy (GDMT) policies and procedures.
