Field LPN, Full-time: Monday - Friday 8 am to 5 pm

The schedule for this role is Monday

• Friday 8 am to 5 pm. This is a full-time professional position. The LPN provides routine and prn visits for assigned patients under the supervision of the assigned RNCM and or IDG Team Leader. The LPN may also provide continuous nursing care for patients during a short-term period of crisis on a rotating basis and provide phone support to patients and their caregivers for their assigned team(s).
