Join a team dedicated to supporting young children and their families through comprehensive behavioral health services. As a Care Coordinator at the Birth to Five Center of Excellence, you'll guide families through enrollment, assessment, and treatment planning, collaborate with providers, facilitate Child and Family Team meetings, and ensure continuity of care.
This role is key in developing integrated care plans, maintaining medical records, and identifying service needs to enhance early childhood outcomes. This is a 32-hour per week position.
