Matrix Home Care is seeking compassionate and experienced Physical Therapists and Occupational Therapists to provide per diem home health services. Position Details Position Type: Per Diem Schedule: Flexible Setting: Home Health Compensation: Competitive per-visit rates Responsibilities Complete comprehensive patient evaluations in the home setting Develop individualized plans of care based on each patient’s condition, needs, and goals Provide skilled physical or occupational therapy services within the therapist’s licensed scope of practice Monitor patient progress and modify treatment plans when clinically appropriate Complete timely and accurate visit documentation Educate patients, family members, and caregivers regarding treatment plans, safety, mobility, home exercises, and independence Collaborate with physicians, nurses, therapy professionals, and other interdisciplinary team members Communicate changes in the patient’s condition to the appropriate clinical team members Ensure services and documentation comply with agency policies and applicable state and federal regulations Requirements Physical Therapist Requirements Active and unrestricted Florida Physical Therapist license Minimum of six months of physical therapy experience Home health experience preferred Strong patient assessment, treatment planning, documentation, and communication skills Occupational Therapist Requirements Active and unrestricted Florida Occupational Therapist license Minimum of six months of occupational therapy experience Home health experience preferred Strong patient assessment, treatment planning, documentation, and communication skills Valid driver’s license Reliable transportation and current automobile insurance Current CPR or Basic Life Support certification preferred Ability to successfully complete an AHCA Level 2 background screening Ability to travel to patients’ homes within the assigned service area Excellent organizational and time-management skills Ability to work independently while collaborating with an interdisciplinary care team Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
