Case Manager- Agape House

Responsible for conducting risk/needs assessments and developing service plans for all clients assigned to their caseload. Schedule and facilitate case management meetings with clients, facilitate scheduled workshops, programs, and groups addressing clients' needs, and develop resources for needed client services.
