Healthcare Liaison (Admissions Coordinator)

We are looking for a Community Healthcare Liaison to manage the Skilled Healthcare admissions process and serves as a key contributor to occupancy growth through timely lead response, professional referral relationship management, and collaboration with Lifestyle Advisors and community leadership. This position is responsible for supporting the department’s activities in pre-admission, admission, discharge, and follow-up processes while driving responsiveness, referral conversion, and prospect-centered experience from first inquiry through move-in.
