Healthcare Coordination Specialist - Englewood Apartments

PAY: $21.23-23/hour. The Healthcare Coordination Specialist II will work with residents to develop appropriate case plans and goals and provide on-going assessment and support. Assists the residents to utilize community resources to meet individual needs. Works with residents to identify their skills and abilities to promote personal growth.
