Registered Nurse - Community Treatment

The Registered Nurse (Community Treatment) provides comprehensive nursing services to individuals receiving community-based behavioral health services through the Assertive Community Treatment (ACT) program. The nurse works collaboratively within a multidisciplinary team including psychiatrists, therapists, service facilitators, and community partners—to support client stability, recovery, and overall health.

Services are delivered in the home and community settings and include medication management, administration of long-acting injectable medications, physical health monitoring, symptom assessment, client education, and care coordination.
