Care Team Specialist - Iowa CCBHC

The Care Team Specialist serves as the behind-the-scenes point person who ensures consistent communication among a client’s care team, both within the organization and with external providers, to support optimal care. They help connect clients to additional resources or programs as needed and are responsible for completing the documentation required for the Certified Community Behavioral Health Clinic (CCBHC).

Compensation

between $19.74 and $23.54 per hour (wage is based upon total years of relevant experience). Work Schedule: 37.5 hours per week
