Oncology Nurse Navigator - Contract Health - Tulsa

The purpose of this position is to provide case management and educational support specifically for cancer patients, focusing on both chronic and transitional care needs. The incumbent monitors patient progress throughout the cancer care continuum, coordinates clinic and community resources, and ensures timely access to services.

Additionally, the role includes delivering cancer education, promoting wellness, facilitating and expediate CHS referrals throughout the Muscogee (Creek) nation Department of Health to improve outcomes and support cancer prevention efforts.
