Job description
The Outreach Specialist provides outreach to patients needing services based on third party payer incentive and value based program requirements. The Outreach Specialist works collaboratively with Quality Improvement Manager and Quality Improvement Director to accomplish outreach and engagement goals. Participates in a team-based approach to care in the Patient-Centered Medical Home (PCMH).
Participates in activities related to Quality Improvement.
