Job description
The ECM Case Manager will be responsible for assisting individuals and/or families who are homeless and have a chronic illness or physical disability or are high utilizers of the health care system with achieving and maintaining health, mental health, and housing stability. The goal in providing these services is to make significant progress towards addressing homelessness, and medical and behavioral health issues for the county’s most vulnerable populations, to ensure they have the resources and support they need to thrive and succeed.
